OB/GYN Office Policies
We request that you arrive on time to your appointment. Please bring your insurance card and copay (if you have one) to every visit. If you are a new patient we will send you information in the mail-please fill this out before coming to your first appointment. Also, you will need your insurance card, copay and your drivers license or picture ID.
If, for any reason, you cannot keep your scheduled appointment we simply ask you to call us. We are usually very accommodating to reschedule that appointment. Our policy is to charge a fee for No Show appointments if you do not call us at least 24 hours prior to your appointment time. We will charge $30 for appointments and $100 for Office Procedures.
After Hours & Emergency Care
We recommend the use of Urgent Care for problems like: pain urinating, urinary frequency or pain that will not resolve with over the counter medication. We refer to Mercy Health Urgent Care at 1471 E Beltline Ave NE, Grand Rapids, MI 49525 and they are open from 8am – 8pm.
The Emergency Room is recommended for problems like: pregnant patients with any bleeding or leaking of fluid, or pain that cannot wait for office hours-this includes painful contractions. We prefer that you use Mercy Health Saint Mary’s at 200 Jefferson Ave SE, Grand Rapids, MI 49503 and they are open 24 hours a day. Dr Dalm has privileges at Mercy Health Saint Mary’s.
If you have an issue that cannot wait until normal office hours please call our office number (616) 247-1700. The answering service will advise you according to Dr Dalm’s direction or get a hold of Dr Dalm, if appropriate, and he will take care of you. For an emergency that requires hospital care please go directly to Mercy Health Saint Mary’s and they will take care of you. However, if your issue can wait for our office staff please call during normal business hours.
Please call our office at (616) 247-1700. We ask for 24 hours to fill your prescription. You will need to supply your name, date of birth, a number to reach you at, prescription name, how you take your medicine, and which pharmacy you would like it called into. If you have the pharmacy number please leave that too.
Please come to your appointment prepared to pay your portion. If you owe a copay for your visit, it is required before you see the doctor. If you have insurance we will bill it for you as a courtesy but you must supply us with the proper information.
